The honest read is that the phrase rarely describes a structural problem. It usually describes a constellation of soft-tissue and posture features that read as low-energy in still photos: chronic mouth-breathing posture, hunched shoulders, slack resting expression, weak lip seal, untreated skin texture, hair framing that does not work with the face shape, body composition that smothers the jawline.
